    Dear Parents & Athletes

    Welcome to the 2024-2025 School Year!! We are excited to get the year started and we look forward to having you join one or more of our sports programs here at Eisenhower.

    Before any student-athlete can begin participating in sports, you will need to get cleared by the Athletic Director. Please scroll down to the Athletic Clearance Process section to see all the requirements. The entire process will be completed online & you will be required to upload any required documents.

    Student-athletes may participate in more than one sport, but it is recommended that the sport(s) be in different seasons. Here is a list of sports by season:

    • Fall Sports: Football, Girls’ Volleyball, Co-Ed Cross Country, Boys’ Water Polo, Girls’ Tennis &
      (Cheer – All year long sport)
    • Winter Sports: Boys’/Girls’ Basketball, Boys’/Girls’ Soccer, Boys’/Girls’ Wrestling & Girls’ Water Polo
    • Spring Sports: Baseball, Softball, Co-EdTrack, Co-Ed Swim, Boys’ Volleyball, Boys’ Tennis

Athletic Clearance Process

  • To participate in sports at Eisenhower High School the following items MUST be completed:

    1. Complete a Sports Physical – MUST use EHS Physical Form, unless physical is complete at Kaiser Permanente.
      • You can print the physical form from the EHS Athletics Webpage.
      • If you DO NOT have access to a printer, you may pick up the Physical Forms at Eisenhower.
      • Physical MUST be signed, stamped and dated by medical office/physician.
    • Once you have completed your physical – hold on to it. You will NOT turn it to Athletic Director, instead, you will upload a picture of your completed physical onto your Athletic Clearance Profile.
    1. Log-in or Create an Online Athletic Clearance Profile
      • Go to www.homecampus.com.
      • Parent/Guardian will need to Register and create an account under Parent/Guardian Email.
        • DO NOT use RUSD student email.
      • Parent/Guardian who already have an account please log in to existing account.
      • Once you are logged into account – SELECT Start Clearance Here
      • Type in School Name & Confirm School Address (1321 N. Lilac Ave. Rialto, Ca 92376)
      • Select School Year (2024-25)
      • Add Sports (If you are or plan to be a multi-sport athlete, please add all sports you plan to participate in)
      • Complete ALL Required Fields
      • Upload the following items onto your Online Athletic Clearance Profile, under Files:
        • Sports Physical (Upload under Physical Form)
        • Medical Insurance Card (Upload under Proof of Medical Insurance)
          **Items can be scanned or you can take pictures. Just be sure the pictures are clear & show the entire document.
        • Proof of Medical Insurance is required to participate in high school sports. If you enter NO to the question “Is the Student Covered by Insurance?” a drop down option will provide a link & instructions to purchase our District Approved Student Insurance. Please follow those steps.

    Confirmation Page – MUST be printed out and signed by Parent/Guardian & Athlete, then returned to the Athletic Director. If you do not have a printer, the Athletic Director can print it out for signature.

        • Note for Returning Athletes: Collecting the Confirmation Page is a NEW requirement. This must be done in order to get cleared.
    1. Official Sports Clearance from the Athletic Director is REQUIRED before starting first practice.
      • Once you have completed ALL the items above, please email or go see the Athletic Director so you can get cleared. Athletic Director’s Email: mmarquez@rialtousd.org

    Other Athletic Eligibility Requirements

    • Student-Athlete MUST maintain a 2.0 GPA or higher to compete in games.
      • Grades will be checked at 1st semester and 2nd semester grading periods.
    • Transfer Students:
      • If you attended another high school and have now transferred to Eisenhower and wish to play a sport, we will need to complete CIF Transfer Paperwork. Please email the Athletic Director to begin the process.
