Health Screenings for Students
Overview
Starting school is a big step in a child’s life. To ensure a good start, it is important that each child has a physical examination, a dental check-up and is current with their immunizations.
Health Screenings
Vision, hearing, scoliosis, dental and growth screenings are provided to all Rialto Unified School District students as required by state law (grade level specific). All screenings are conducted by one of our RUSD Licensed Registered Nurses.
Kindergarten Physical Exam
California’s Child Health and Disability Prevention (CHDP) law requires every child to have a physical examination before entering school in kindergarten. Parents/guardians are required to present a report of examination or a waiver statement to the school within 18 months prior to entry into first grade or within 90 days of entry into first grade. Obtain the form here or from the school’s health office.
Students who have not met this requirement by the appropriate date may be excluded from school attendance. Parents/guardians needing assistance meeting the requirements should call your school for additional information. You are encouraged to complete this exam at kindergarten entry to make sure your child is healthy and ready to learn.
K-1 Dental Exam (OHA)
California law, Education Code Section 49452.8, requires an Oral Health Assessment (dental check-up) by a licensed California dental health professional for children entering public school for the first time (at kindergarten or first grade). The Oral Health Assessment must be completed and returned to the school by May 31 of that first school year. Assessments that have occurred within the 12 months before your child enters school also meet this requirement. Obtain the form here (Spanish) or from the school’s health office.
Parents/guardians may sign a waiver of this requirement, which is available at the school. There is no penalty of exclusion if this requirement is not met. If you cannot take your child for this required assessment, please contact the school nurse or health clerk at your child’s school health office. You can also contact your local County Social Services office for more information on preventive care services for eligible low-income children and adults.

Immunizations for ChildrenÂ
California State law requires that all students under age 18 years, pre-kindergarten through grade 12, be immunized against certain diseases unless they are exempt for medical reasons or because of personal beliefs. At the time of registration, proof that your child is fully immunized, is up to date with immunizations, or if your child qualifies for an immunization exemption is required by the school.
Information on obtaining a permanent medical exemption or personal beliefs exemption for your child is available from the school nurse.
T-dap Information
Effective July 1, 2011, a new immunization requirement takes effect for student attending 7th through 12th. The state is now requiring that students be immunized against pertussis, or whooping cough, as a condition to attend school. By State Law, students will not be allowed to attend school without this immunization.
Immunization Clinics
Rialto Unified School District offers free immunizations through the VFC Program to all school-age children that meet eligibility requirements at the Health Services office on the 2nd and 4th Tuesday and Thursday of the month. Please call to schedule an appointment prior to bringing in your child for any immunizations.
If you child is eligible to receive immunizations from RUSD, parents/guardians need to bring the following documents to the appointment:
- Parent/Guardian ID
- Proof of Guardianship
- Immunization Card
The district participates in the California Immunization Registry (CAIR), a countywide computer system that keeps track of immunizations (shots). See the Immunizations box to the right for specific requirements
*Please note: If your child has medical insurance, please take your child to see their assigned pediatrician, family physician or medical clinic for immunizations or to make sure your child is fully immunized.

Home HospitalÂ
Overview
Home Hospital is a temporary alternative to school instruction and is available to students who cannot leave their homes to attend school because of serious medical problems such as leukemia, surgery or other illnesses as deemed appropriate. Behavioral issues need to be assessed by Special Education for an appropriate placement.
Eligible students in Home Hospital are assigned a RUSD Certificated Teacher that is credentialed in the course(s) assigned.
Requirements
The student’s medical provider must provide a written request for Home Hospital and included in the request the provider needs to state the reason/diagnosis and duration of time; referral not to exceed forty-five (45) days. Beyond any forty-five (45) day period, a new completed referral is required for services to continue.

First Aid and CPR Classes
Overview
Health Services is currently only providing training for Rialto Unified School District employees and whose job description requires a CPR and/or First Aid certification. Classes are instructed by an in-house Licensed Registered Nurse certified by the American Red Cross.
There is no cost to Rialto USD employees whose job description requires a CPR/First Aid certification; otherwise, for all others, a $29.00 fee is due prior to the scheduled class to be attended. Forms of payment accepted are cash or check; if paying with cash, payment must be in exact change.
Please note: Employees who register for a CPR/First Aid class and fail to attend the training will be required to pay a $29.00 fee for the next class at the time of registration (non-refundable from Health Services). Personnel Services will reimburse employees $29.00 after completion of the CPR/First Aid Class.
CPR and First Aid certification are valid for 2 years from the date the class was taken.
All classes are held at the Health Services Training Room located at 815 S. Willow Avenue, on Thursdays from 4:30 P.M. to 7:30 P.M. (see class schedule below).

- Employee must register for the training in advance; attendance to all CPR and First Aid classes are on a first-come-first-serve-basis.

Tuberculosis (TB) Skin Tests
Health Services is currently providing Tuberculosis (TB) Testing for all Rialto Unified School District employees and for those who are in the process of applying to become an RUSD volunteer.
